Who is the service for?
Adults with learning disabilities.
What do they do?
The Community Team for People with Learning Disabilities (CTPLD) is a joint health and social care team that supports people with learning disabilities to lead an independent life as much as possible and have access to health services, so they can be as healthy as possible. The team also supports family and paid carers to continue to support people with learning disabilities and health professionals to help people to access their services.
How can I access the service?
All new health referrals should be sent to the Common Point of Entry team at:

Alternatively, any person can make a referral by phoning the CPE team on 0300 365 0300.

Out of hours, during weekends and on bank holidays, support can be sought from Social Services Emergency Duty Team or GP Out of Hours services.
