Who is the service for?

There are services in east and west Berkshire for patients with Chronic Obstructive Pulmonary Disease (COPD).

East Berkshire COPD Early Supported Discharge and Admission Advoidance Service

What do they do?

The COPD Early Supported Discharge and Admission Avoidance Service assesses people both before they are discharged from hospital and whilst they under the care of the service, to ensure that their medical and nursing needs are addressed. The team also provide a rapid response service, helping patients to manage exacerbations (flare ups) and offer them support at home to help prevent admission to hospital.

The team is made up of a variety of specialist practitioners who work across both primary and secondary care to offer COPD patients in the east of Berkshire support with their disease and to reduce future hospital admissions.

The service offers patients:

All patients who are referred to the service must have a confirmed diagnosis of COPD and should have had recent spirometry.

The COPD Early Supported Discharge Service can provide oxygen and nebuliser loans as patients require.The team has close links with the Home Oxygen Service - Assessment and Review to provide an integrated approach to care including safety assessments and six month and annual reviews for patients receiving oxygen.

Where can I access the service?

Our COPD Early Supported Discharge Service is based at the Chest Clinic at King Edward VII Hospital, Windsor. The team may see patients in a variety of care settings, including acute (large) and community hospitals, or in a patient's home where necessary.

West Berkshire Community Respiratory COPD,
Pulmonary Rehabilitation and Home Oxygen Assessment Service

What do they do?

The service provides pulmonary rehabilitation, home oxygen assessment and a rapid response service for those patients with an exacerbation of their COPD. See our service leaflet here.

The service offers patients:

The team is made up of respiratory nurses, respiratory physiotherapists and administrators and the service will cover the West of Berkshire and support patients with their disease and reduce future hospital admissions.

All patients who are referred to the service must have a confirmed diagnosis of COPD and should have had recent spirometry.

The rapid response service can provide nebuliser loans as the patients require. The service works with many other agencies to make certain that the most appropriate care is given to the patient from those best placed to provide it.
